Completing your Application

These are the forms GMA must have on file for you for your student to be accepted:

• Parent Interest/Involvement form
• GMA Pre-Application form
• Three Reference Forms
• Birth Certificate
• Immunization Record (waiver option included)
• Health Examination form or waiver
