Mentor Form #1

Module 4 Mentor/Student checklist (pdf) (word)

Mentor and Student both download the checklist use as you complete the module, and sign and return at the end of the Module.

All students must complete the TILT if they have not yet done so by the end of the module in which they are currently studying. The link to this required tutorial is as follows: http://tilt.lib.utsystem.edu/intro/registration.htm
You will have to register with your name and e-mail address to use this website When prompted at the end, please enter services@wciu.edu to have the results of your quizzes sent to WCIU.

Mentoring Sessions (pdf) (word)

Fill this form out after every mentoring session Be sure to save these forms until the end of the half-module, at which time you will submit them all

Mentor Form #3:

Mentor Module 4 Assessment (pdf) (word)

Fill this form out at the end of Module 4, and submit it along with the Assessment Keys that you filled out for every mentoring session

Student Form #1

Student Form #2:

Graduate Research Paper Topic
Approval Form (pdf) (word)

The Student must have the Topic of their Graduate Research Paper Approved before 6 months have passed while enrolled in Module 4 and before beginning Module 4 Lesson 41. Use this form to submit your topic

Student Form #3:

Graduate Research Paper Outline and Bibliography Form (pdf) (word)

The Student must submit the outline and the bibliography of their Graduate Research Paper before 6 months have passed while enrolled in Module 4 and before beginning Module 4 Lesson 41. Use this form to submit your Graduate Research Paper Outline and Bibliography.

Student Form #4:

Graduation Application (pdf) (word)

The Student must apply early in Module 4 to graduate from the Global Civilization program.

Student Form #5:

Student Course Feedback (pdf) (word)

Fill this form out at the end of this module to give feedback on the study program

Capstone Project

In Module 4 the student is asked to produce a Capstone Project.  Please read through the Capstone Project Guidelines (linked to below) carefully.  Begin thinking about and working on your Capstone Project early.

will be sent to the mentor when the approved Enrollment Agreement is
returned.
