
Five Easy Steps to Enroll in a Module

Step 1. Download the form Enrollment Agreement:

It is preferred that the Enrollment Agreement be filled out electronically in the Microsoft Word document.
Please return both pages of the Enrollment Agreement. The second page is what you are agreeing to.Step 2. Complete the form
A. Complete the contact information at the top of the page.
B. Enter an “X” in the checkbox for the appropriate Module.
C. Enter the Proposed Start Date
The Proposed Start Date can be the first day of any month after the Admissions and Records Office receives the form from you. If this is your first enrollment, see your Admissions letter for the deadline for you first enrollment. Be sure you will have your books. Allow two weeks to process your book order, plus shipping time.
D. Enter the Proposed Completion Date as the first day of the month, six months after the Requested Start Date.
E. Enter the amount owed.
1. Enter $480 tuition for a half module, or $960 for Module 4.
2. Enter $120 for the nonrefundable fee for a half module, or $240 for Module 4.
3. Enter any scholarship amount to subtract.
4. Enter the total.
5. Enter the total amount the student is responsible for.
F. Read the information on the back of the form.
G. Sign the Form
Step 3. Obtain your mentor’s signature and send the form to WCIU.
Option A.
It is preferred that the document be sent by email. In lieu of your signature, you may send a statement of agreement to your mentor. The mentor may then send the form, along with your statement, to the Registrar's Office.
If you choose this option, your e-mail statement should state the following:
"I understand that the school catalog and its contents are binding on the school and me. My email statement certifies, in lieu of my signature, that I have read, understood, and agreed to my rights and responsibilities on the second page of the WCIU Enrollment Agreement, and that the university's cancellation and refund policies have been clearly explained to me."
Send your mentor your e-mail statement with the completed document attached. Ask him or her to forward the document to the Registrar by email, and to include the header of your e-mail, as well as your statement.
In addition, the mentor will need to include his or her own statement, "I agree to the student's enrolling in this course," and mail to: registrar@wciu.edu
Option B.
Obtain your mentor’s written signature and Fax the form to the number indicated on the form.
Option C.
Obtain your mentor’s written signature and send the form by postal mail to the address indicated on the form.
Step 4. Send payment to Registrar’s Office.
Option A.
Call with your Visa or MasterCard information to 626-398-2141
Option B.
Send a check or money order to WCIU Registrar 1539 E. Howard Street Pasadena CA 91104. If you choose this option, please call 626-398-2141 or email registrar@wciu.edu to let the office know the check is on it’s way.
Step 5. Download and Sign the Statement of Academic Integrity.
Option A. It is preferred that this Statement be received as an email statement. Just copy and paste the statement into an email and send it to registrar@wciu.edu. Please put in the subject line
“Statement of Academic Integrity Module (indicate module number)"

Option B. Download the form, sign in writing, and mail the form to WCIU Admissions and Records 1539 East Howard Street Pasadena Ca 91104 or fax to 626-628-3234.

Once the Enrollment Agreement, Statement of Academic Integrity, and payment are received (assuming that previous modules have been finalized), mentor and student will receive an approved copy of the Enrollment Agreement, and the mentor will be mailed a link to download language quizzes and tests.
